Initial Physio Assessment: $150 ($180 in Hospital)
Follow-up Physio Treatment: $120 ($150 in Hospital)
Work and Home Ergonomic Assessment: $200
​At-Home Nutritional Assessment: $250
Nutritional Consultation Follow-up: $150
If you have extended health insurance I will provide you with a receipt that you can submit to your insurance provider. 
​
A doctor's referral is not required unless your extended health insurance plan requires it for reimbursement.
If you live outside of the Kingston area travel expenses may apply. This will be established prior to the appointment.
